Winter Coat & Boot Request

Please fill out the form below for each child you are requesting a coat and boots for.  Requests must be made by November 3rd to ensure we can fulfill all requests.
Boots, coats and toys will need to be picked up on Tuesday, December 13th from 9:00 am to 7:00 pm
